Project Description:

Increase collaboration between general practice, including the Aboriginal and Islander health Service, other allied health professional and community organisations dealing with MH and AOD comorbidity issues. Improve the skills, knowledge and capacity of GPs, practice staff, allied health professionals and community organisations in the early intervention and management of drug and MH comorbidities in primary care. To improve local coordination of service provision including share-care models with a focus on early intervention strategies that will reduce the demand on specialist services over the longer term. To develop share-care referral pathways with relevant services with particular emphasis on referral and discharge information exchange with an aim to progressing to electronic communication and standardised processes in the future. Establish and maintain a local service provider directory with links to referral pathways. Update of the MoU between the division, and the AOD local service providers.
