Name of Project: Comorbidity Networks Project
Project Description:
To bring together community service providers working with clients with comorbidity issues. This will enable development of a collaborative approach to a treatment model linking the primary health sector with the community services sector. Establish a pathway through which community service providers and primary health services can cooperate strategically and operationally whilst maintaining the GP's central role in managing the care of clients. Establish interagency protocols and referral pathways to enhance and better manage comorbidity. Promote professional development across agency workforce to provide a better understanding of the changing therapeutic environment.
