Name of Project: Development of a Web Based Mental Health Referral System
Project Description:
Maximise opportunities for GPs to be involved in multi-disciplinary care through provision of a town based directory of allied mental health workers available. Promote timely and appropriate pathways of care for clients with comorbidity issues through provision of up to date and relevant information of services available, specific to areas. Promote efficiencies across the administration of program delivery. Promote efficiencies across the primary health care networks.
