Name of Project: Comorbidity Service Integration Program

Project Description:

Fostering between-service networking through focused planning meetings between key stakeholders. These meetings will be used to map services involved in management of patients with AOD and MH comorbidities, and establish MoU that outline local referral and case management protocols between services sharing the care of these patients. Working relationships and communication protocols between organisations will be determined. What will follow is multidisciplinary case discussions between service providers involved in treating the same case, and case conferences that involve service providers and patients and their Carers/families. This will result in improving knowledge regarding the region's AOD and MH services and facilities, improving understanding of how to cross-refer appropriately between the various services, enhancing follow up care, and establishing and maintaining clear pathways of communication.

Develop locally appropriate information for consumers, families and Carers, aimed at educating them regarding the signs and symptoms of AOD and MH comorbidity, and outlining local referral pathways and treatment options available. Establishment of family and Carers support groups at the local level, based in local community services interacting with GP practices and the division.
