Name of Project: Cat1: General Practice Mental Health and Substance Use Comorbidity Engagement and Education Program

Project Description:

To increase the capacity of the division staff to educate on comorbidity prevention, early intervention, treatment and management. To establish a team of Can Do facilitators in CW Gippsland.  To build on success of Can Do, focusing on GP engagement, education, knowledge and work practices. To provide financial support for general practice staff to participate in Can Do networking units. To indentify and support general practice "champions" in identifying, treating and managing co morbid patients. To undertake a priority Can Do networking project with the CW Gippsland Aboriginal Health and Housing Co-Op. To improve health outcomes for co morbid patients, especially young indigenous people.

Name of Project: Cat 2: An integrated Mental Health and Substance Use Service System for Central West Gippsland

Project Description:

To provide coordinated care for young people with MH and substance use issues by developing local referral policies and protocols. (generic and indigenous). To improve health outcomes for co morbid patients, especially young indigenous people, through coordinated care and systematic change at the local level. To enhance the relationship between MH services, AOD services, general practice and youth services. To build on the success of the Can Do initiative, focusing on supporting and maintaining networks. To support general practice "champions" in participating in interdisciplinary networks.
