Name of Project: Navigating the road to change - primary care partnerships for comorbidity

Project Description:

To enhance early and accurate ID of patients who have a co-morbid presentation through adopting a multidisciplinary shared care case conferring /coordination approach in a primary care setting.  A whole of practice or system-based training and induction process will be developed to improve patient care. Cross-sector collaboration with key players in the sector will be used to enhance formal structures and networks to develop improved frameworks which build local service delivery capacity. These will also enhance knowledge and skills within a multidisciplinary framework, and establish a robust local comorbidity infrastructure.
