Project Description:
Advocate and promote a multidisciplinary approach to early ID, early intervention and treatment of patients with a substance use and MH need. Identify and promote the evidence-based screening tools with regard to shared care between GPs and others. Develop visual aids for GPs and others in the referral process. Improve pathways of care for patients. Improve the level of awareness of those engaged in treatment of patients with MH/AOD needs. Eliminate barriers to access and treatment between services providers through communication strategy. Create a platform for education for GPs and clinicians. Communicate complimentary and competing initiatives proposed and implemented in each state. Improve the clinical relationships which lead to more effective communication and more targeted patient referral.

Name of Project: Perinatal Co-morbidity Initiative
Project Description:
Deliver the Can Do unit on drugs, sex and pregnancy to GPs, specialists, allied health, pharmacists and others, a minimum of 2 lots of 6 hours of education over the next 12 months. Create and maintain coordinated pathways to shared care. Maintain effective communication between all services supporting and treating the target group. Help with the establishment of a primary health care model that identifies, screens and treats perinatal patients and families with comorbid conditions. ID and eliminate barriers to access and treatment between service providers. Identify pre-existing risk factors for this group and identify strategies to reduce the risk of incidence and harm. Find and distribute educational and preventative resources for this group. Contribute to community education and community development for perinatal care impacted by the issues of substance use and mental health, focusing on young people and their families.
