Name of Project: Comorbidity Forum
Project Description:
Strengthening links in the GP community by the following activities. Consultation among practitioners involved in complex care cases to ensure barriers to care provision are identified and minimised to allow effective case management of individual clients. Provision of five forums which will allow opportunities for pertinent information to be discussed. Provision of a newsletter to the sector to improve information dissemination in the sector. Provision of support for integration of network resources, network navigation, and support.
